Requirements for Filing Design Application:
Filing
1. Scan copy simply signed Power of Attorney;
2. Drawings of design (front view, rear view, top view, bottom view, right side view, left side view, perspective view);
3. Brief description of the design;
4. Scan copy simply signed Declaration of Ownership signed by applicant;
5. Scan copy simply signed Assignment of Right (if the application is filed not by the designer);
6. Certified true copy of priority application (if any).
Recording change of name/address
1. Scan copy simply signed Power of Attorney;
2. Document of change of name/address.
Recording assignment
1. Scan copy simply signed Power of Attorney;
2. Deed of assignment notarized and legalized up to the Indonesian Embassy/Consulate in the assignee’s country;
3. Copy of design certificate.
Recording license agreement
1. Scan copy simply signed Power of Attorney;
2. Copy of license agreement;
3. Copy of registration certificate.
